$1,000 Yearly Membership

• 1 year commitment required.  Payments made via auto pay either monthly, quarterly, bi-annually, or one time full

• 10% discount for annual payment ($900)

• 5% discount for bi-annual payments ($475 every 6 months)

• Quarterly ($250 every 3 months)

• Monthly ($83.33 every month)

• An additional 10% discount is given for the 3rd or more family member(s) when paying the full 1 time payment option ($800.00)

• An additional 5% discount is given for 3rd or more family member(s) when paying the bi-annual payment option ($450.00 every 6 months)

Membership includes all of the following benefits

•   Unlimited access during normal business hours.  (Monday-Thursday 9am – 5pm)

•   In office and telehealth appointments. (1 in office appointment is required per year)

•   Annual, FDA approved, Qb computerized testing. (as needed)

•   Urgent phone calls, virtual appointments and/or email communications with Dr Brugman during normal business hours.

•   Monthly prescriptions printed or electronically sent to your pharmacy.

•   Pharmacy change accommodation as needed.

•   Prior authorization completed for medications.

•   Priority access to medical and prescription questions.

•   Priority accommodation for the completion of forms and letters.

•   Secure text messaging.

•   Collaboration with schools, therapists, psychologists and/or psychiatrists.

•   Superbills provided, upon request, for you to submit to your insurance company.

New Patients ADHD Evaluations $400 

(You are a New Patient if you have not had an appointment with Dr Brugman in the last 2 years)

• Testing, Evaluation and Consultation with Dr Brugman

•  Comprehensive ADHD evaluation including FDA approved computerized testing. (Qb test for ages 6 to 60)

• Standardized behavior rating scales.

• 1 to 2 hour diagnostic interview.

• Detailed review of medication and non-medication treatments.

• Diet and nutrition advice.

• Time to have all your questions answered.

• 1 Follow up appointment for purpose of re-testing on medication within 4-6 weeks following first appointment if medication is prescribed.  

Please note: This follow-up appointment is solely for re-testing on medication, if medication is prescribed and taken as directed.  It is not a guaranteed future appointment for further consultation or evaluation.  Charges will apply if another consultation visit is needed.

Pay Per Visit Option: $400.00

• Patients must be seen at least every 3 months to receive continued medication management.

• A maximum of 1 prescription will be provided for this type of visit.

Membership can be canceled with a written request. Balance of the one-year membership cost will be due upon our receipt of written cancelation. Once membership is canceled, the full year membership payment ($1000.00) is required to be reestablished as a patient.

Credit Card or HSA card will be kept on file.

We do not file insurance.  You can self submit a claim to your insurance company.  Dr Brugman is out of network for all insurance companies.  Claim acceptance is not guaranteed by Brugman Attention Associates.
